Program Structure

The Health Coach will meet with the patient biweekly as well as asynchronous communication through the patient’s mobile app.

Each week will be dedicated to that week’s lesson that pertains to one of the elements of the COR-4, however, the coach and patient will focus on behaviors that are most important to the patient. The program does not need to be followed sequentially; although, it is recommended that the patient read the content in order.

The healthcare provider will meet with patients monthly. For patients who are on GLP-1 treatment, they may need to be seen more frequently.

Health coaches will meet with their patients ~bimonthly. They will maintain close contact with their patients through asynchronous communication as well as short, telehealth visits.

When the program has been completed, meaning that all lessons are finished, the patient will have the option to continue in a maintenance program (PowerME NEXT). Coaching typically continues at a reduced frequency, but should involve shared decision making. Similarly, meetings with their health provider will often be reduced in frequency. It is during this period that the patient gradually reduces their medication dose (tapering). 

Once that patient no longer having active coaching (even if they continue to see their provider for medical management), the program is deemed completed and the patient is inactive.

All patients who have completed the program are encouraged to check in periodically with their coach and provider and to help with ongoing maintenance and to help prevent weight regain. Most patients will benefit from these periodic ‘tune ups’.

The health coach’s main role is that of a coachnot strictly an educator. Health coaches follow the AMT framework: accountability, motivation, and troubleshooting.

 The healthcare provider’s role includes medical management of GLP-1 treatments, reviewing the lab values and mental health surveys with patients, counseling and education, as well as providing accountability and motivation.

  • The program features telehealth appointments with the patient’s PowerME healthcare provider and health coach, as well as asynchronous communication and telehealth visits with their health coach.
